If you earn commission income or pay employment expenses, such as buying your own tools or using your own vehicle for work, please download this form and have your employer complete and sign it.
Note: YOU CANNOT CLAIM EMPLOYMENT EXPENSES IF YOU DON’T HAVE FORM T2200 SIGNED BY YOUR EMPLOYER.
+T2201 Disability Tax Certificate (For Individuals with Long Term Disability)
If you suffer from long term physical (more than 12 continuous months) or mental impairment you are entitled to the disability tax credit. Please download form T2201 and take it to your doctor to fill out and certify it. Then forward it to our office.
+RC-199 Voluntary Disclosure Program Application (for Individuals who have not filed taxes for several years)
If you have not filed a tax return for several years you should complete part 1 and sign part 4 of the Voluntary Disclosure Program Application and forward it to our office. If your application is accepted CRA might waive filing penalties and interest once you file your returns.
